Client, Family and Caregiver Satisfaction Survey

We want to hear from YOU! Are you a current or past client of CMHA? Or a parent or caregiver of a client of CMHA? Help us improve the services we provide by completing the Ontario Perception of Care Tool (OPOC) for Mental Health and Addictions.

This survey is anonymous and can be done online. To get started, please complete the form below and we’ll then send you a link to the survey and a code that allows you to get started, or send you a paper survey if you prefer.

This survey is being used across the Province of Ontario, by many mental health and addictions agencies. The information you give helps us observe trends in our community and beyond, improve service and respond more quickly to community needs.
