If you are working with a doctor, nurse practitioner or other health care provider, they can also refer you using the referral form on our website. The Mental Health First Response Team is available for people who may be experiencing mental health problems or crises and is also available for people who are looking for guidance on how to support someone close to them. Services include telephone crisis intervention (mental health assessment, suicide intervention), referrals, and family support. This team is also available to healthcare providers seeking assistance navigating community resources.
MobileCare, Community Health Outreach

MobileCare is a clinic on wheels that travels to both urban and rural communities and locations across Chatham-Kent and Sarnia-Lambton. With our partners in each county, our goal is to increase access to primary care, mental health care, addictions services, withdrawal management services and community referrals. MobileCare is a free walk-in service, no appointments are necessary.

Chatham-Kent Youth Wellness Hubs Ontario & Sarnia Lambton Youth Wellness Hubs Ontario

For youth ages 12 - 25, the Youth Wellness Hubs are welcoming, youth-friendly spaces with services for young people and their families who are looking for supports related to mental health and substance use concerns.
You can reach out for support just by walking in or contacting the hub by phone or email. Our team of professionals are here to provide mental health, substance use, peer support, education, employment, wellness, and so much more. At your first appointment, we will spend some time getting to know you and we will get a better understanding of your needs through an assessment. From there, our team will work with you to determine the programs and services that will best meet your needs.

Centralized Assessment and Referral
CMHA provides an initial assessment and a single point of access, for all community-based adults (16 years and older) mental health services in Chatham-Kent. Referrals are accepted from all sources (i.e. self-referral, family, friends, physicians, clergy, community service providers, etc.). At times when CMHA Services are determined not to be required for the individual in question, referrals to other community services will be made whenever possible.

Clinical Case Management
Bio-psychosocial Rehabilitation emphasizing client choice, empowerment, and individual strengths, is provided on a one-to-one basis. Services include care planning, problem solving, coaching on therapeutic lifestyle choices and education on health and medication. Advocacy is also provided in accessing financial benefits, housing, food, clothing and community health services. Case Management also provides crisis intervention, consultation and coordination, monitoring, support and life skills. Additional case management specializations include: Vocational, Concurrent Disorders Case Management, and Dual Diagnosis.

Discharge Planning from Hospital
Provides assistance to referred individuals who are preparing for discharge from the Mental Health In-Patient Unit at Bluewater Health. Assistance may include help with housing and finances, as well as referrals to appropriate community resources.

Early Detection and Intervention Services (EDIS)
Early intervention is key. EDIS provides early intervention services to individuals between the ages of 14-35 who are experiencing a first onset of Psychosis. Services include: screening, nursing assessment, comprehensive assessment, linkage with a psychiatrist and other community supports, individual treatment planning, reintegration support, family engagement, community awareness and education.

Family Support Program
The CMHA team recognizes that engaging family members is a crucial part of the treatment process. Engaging with family members improves the assessment process, provides the relative with positive supports, will promote a better recovery and decreases the occurrence of relapse. The Family Support Program provides individual family intervention to families of persons who have a loved one who may be struggling with a mental illness.

Geriatric Mental Health Outreach Team and Behavioural Supports Ontario (BSO)
With the goal of enhancing quality of life for older adults with complex mental health needs, this team is comprised of a Psychiatrist, Registered Nurses, and Social Workers. They provide psychiatric evaluation and care within long-term care settings and they work collaboratively with partners to support clients with stabilization. Members of this team also deliver care as members of the ‘Behavioural Supports Ontario’ Lambton County Lead Team. Referrals accepted from Primary Care.

Housing: Supportive Housing
Guided by the Housing First philosophy; a recovery oriented, evidence-based intervention model that involves the provision of permanent housing and/or emergency housing providing wrap-around supports to individuals who are homeless and living with mental illness.

Indigenous Services
The CMHA Indigenous Team provides culturally sensitive Mental Health Services to Indigenous people. Indigenous staff provides services tailored to the individual needs of each community. This multidisciplinary team is comprised of Social Workers, Registered Practical Nurses, and Social Service Workers. CMHA staff partner with existing supports in the communities of Kettle & Stony Point, Aamjiwnaang, Walpole Island and Delaware Nation as well as the Sarnia Lambton Native Friendship Centre.

Justice Services
Support is provided to individuals who have a mental illness or addiction concerns and are involved with the Justice System. When indicated, the CMHA worker can assist by advocating for court diversion. When court diversion is not possible, the CMHA worker can provide support and guidance in navigating the court system.

Lambton HART Hub

The Homelessness and Addiction Recovery Treatment (HART) Hub includes three multi-partner care pillars; a Centralized Hub with MobileCare outreach, Transitional Addiction Recovery Beds and Supportive Housing (fixed site and scattered site). Each care pillar of enables access to primary care, mental health and addiction treatment, housing supports, and other services such as employment assistance, food security and personal hygiene for individuals who face complex barriers in accessing care.
Long Acting Injection Clinic (LAI)
Administration of psychiatric medication, assessment and monitoring of individuals receiving injections and short-term medication monitoring in office or in the community. Metabolic syndrome monitoring is also provided, which includes monitoring the physical health effects and psychiatric medications.

Mental Health and Shared Services Team (MHASST)
Developed through a partnership with Lambton County Shared Services, the team provides brief service to adults (16+) in receipt of Ontario Works funding. They assist with accessing services and supports to address complex needs including social determinants of health, poverty, homelessness, mental health and substance use concerns.

Mental Health Engagement Response Team (MHEART)
A partnership with CMHA, Bluewater Health, OPP and Sarnia Police, the team is comprised of a Registered Nurse or Social Worker who travels with police to provide a rapid mobile crisis response to individuals who have mental health and/or substance use concerns. The team will engage individuals in crisis, de-escalate situations and provide an assessment that connects people with needed treatment and resources. The team also facilitates hospitalization, as needed.

Mental Wellness Centre & Recovery College

A welcoming, peer-driven space where adults living with mental health concerns can find connection, understanding, and practical support along with free, community-based courses designed by and for people with lived experience of mental health and/or substance-use challenges.

Ontario Structured Psychotherapy (OSP)
Ontario Structured Psychotherapy (OSP) offers free, short-term, evidence-based cognitive behavioural therapy (CBT) for adults experiencing depression, anxiety, and anxiety-related concerns.

Psychotherapy Program
The Psychotherapy Program provides a time-limited, Cognitive Behavioural Therapy (CBT) approach to managing depression and anxiety. CBT helps the individual to increase their awareness of negative thought, feeling and behaviour through self-monitoring and psychoeducation. The focus of this service is on areas of illness education, symptom management and on wellness and relapse prevention planning.
The Psychotherapy Program also provides a 20 week Dialectical Behavioural Therapy (DBT) Skills Group. The DBT Skills Group is for those who have self-harming behaviours and for those who struggle with managing their emotions. The Group focuses on building skills within four areas: mindfulness, emotion regulation, interpersonal effectiveness, and distress tolerance.

Rapid Assessment Intervention and Treatment Program (RAIT)
The RAIT team provides unique mental health services in partnership with Primary Health Care Providers. RAIT services are provided to individuals who have limited access to mental health services. The interdisciplinary team will complete an assessment and develop a treatment plan in collaboration with the individual and their Primary Health Care Provider. The RAIT team is comprised of psychiatrists, nurses, social workers, psychotherapists and client care assistants.
