Are you a health or social service provider who would like to refer a client or patient to Canadian Mental Health Association Lambton Kent? Please complete the referral form below and fax it to the appropriate office.
We also accept self referrals and referrals from family and friends. For more information, please contact us from Monday to Friday from 8:30am – 5:00pm in Chatham at 519-436-6100 or in Sarnia at 519-337-5411.
